Isolated laryngeal symptoms as a stroke presentation are extremely rare due to the intimal anatomical relation of swallowing-related structures with the ascending/descending tracts in the lateral medulla [2]. Herein we present a case with isolated vocal cord paresis as the sole manifestation of acute ischemic stroke.
