- Anorectal melanoma: a rare entityPublication . Relvas, Luís Miguel; Gago, Tânia; Velasco, Francisco; Barros, Sónia; Carvalho, Isabel; Caldeira, PauloAnorectal melanoma is a rare malignant tumor with the potential of simulating a benign anorectal disease, making its diagnosis difficult. We describe a case of anorectal melanoma, in which the interpretation of symptoms as hemorrhoidal disease delayed diagnosis and appropriate intervention.
- Gastric mixed neuroendocrine/non-neuroendocrine neoplasia: a rare entityPublication . Costa, José Damasceno e; Costa, Rita Seara; Moreira, Hélder Deveza; Ferreira, AníbalGastric neoplasms have different histological subtypes. Mixed neuroendocrine/non neuroendocrine neoplasms (MiNEN), as defined by the World Health Organization (WHO) 2019 Classification of Tumours of the Digestive System, are rare composite tumours bearing morphological characteristics of more than one histological type. Historically, the diagnosis requires that each component accounts for at least 30% of the total tumour burden. We present the case of a 70-year-old male, who was referred to our endoscopy unit for resection of a depressed lesion (Paris classification type 0 IIc) of 40mm in largest diameter, located on the lesser curvature of the gastric body. The lesion was submitted to en bloc resection by endoscopic submucosal dissection. Histological examination revealed a poorly differentiated intramucosal adenocarcinoma and a grade G3 well differentiated neuroendocrine tumour (NET), on a background of chronic atrophic gastritis. Unfortunately, examination of the NET component revealed intercepted margins. The patient is currently undergoing tumour staging and will be discussed by a multidisciplinary team to determine subsequent management.
- Pantoprazole-induced liver injury in the setting of diabetic ketoacidosisPublication . de Oliveira, Raquel; Almeida, Manuel; Lavado, Pedro; Baptista, AlexandreCritically ill patients are at higher risk of acquired liver in-jury, given the multiple coexisting potential causes of injury.1They are also at risk of stress ulcers, and prophylaxis with proton pump inhibitors (PPIs) is common in Intensive Care Units (ICUs). A 54-year-old woman was admitted to the ICU due to diabetic ketoacidosis (DKA). On admission, she was hemo-dynamically stable, with a Glasgow Coma Scale score of 7 (E2V1M4). Her abdominal examination was normal, without palpable organomegalies, and her liver blood tests were within the normal range. She was intubated for airway pro-tection and started on intravenous fluids, insulin perfusion, and prophylaxis with intravenous pantoprazole 40 mg/day.
- Possible role of mycotoxins, malnutrition and MECP2 dysregulation in nodding syndromePublication . Catamo Vaz, Deise Haua da Silva; Costa, Maria Helena da; Buque, Helena Agostinho; Nzwalo, HipólitoWe read with great interest the manuscript by Spencer et al. [1] suggesting a possible role of Methyl-CpG-binding protein 2 (MECP2) dysregulation in the pathogenesis of Nodding syndrome (NS), a childhood onset epileptic disorder occurring exclusively in sub-Saharan Africa. The sociodemographic and phenotypic overlap of NS with MECP2 duplication syndrome, namely the age of onset interval, female predominance;
