A infeção por Clostridium difficile é uma IACS que afeta cada vez mais as instituições
prestadoras de cuidados e as pessoas ao provocar sintomatologia que pode ocorrer na
forma ligeira ou moderada, com uma apresentação clínica menos prejudicial para a
pessoa, ou sob a forma grave, provocando complicações e até morte.
Para que a sua ocorrência seja evitada é essencial reconhecer os fatores de risco que
predispõem para o seu desenvolvimento, a sintomatologia, os métodos de diagnóstico e o
tratamento mais adequado.
A presente investigação teve como objetivo a avaliação os custos associados à infeção por
Clostridium difficile do Departamento de Medicina do CHUA, no ano 2015. Neste estudo
descritivo do tipo caso-controlo, analisou-se a presença de fatores de risco nas pessoas
que desenvolveram a infeção, os dias de internamento e os custos com terapêutica
medicamentosa e meios complementares de diagnóstico aplicados a 45 indivíduos com
infeção (casos) e 45 doentes sem infeção (controlos).
Com esta análise pode-se concluir que os doentes com a infeção apresentaram uma média
de internamento cerca de 4.1 vezes superior às pessoas sem a doença. Por outro lado os
custos com terapêutica medicamentosa, meios complementares de diagnóstico e,
consequentemente, os custos totais, foram cerca de 3 vezes superiores nos indivíduos que
contraíram infeção do que do que nos indivíduos controlo.
Mediante os resultados, verificou-se que é essencial a consciencialização dos
profissionais de saúde e gestores para esta problemática, sendo fundamental a adoção de
práticas preventivas desta infeção bem como a existência de métodos de imputação de
custos individualizados nas unidades de saúde, que permitam a identificação dos gastos
incorridos.
Clostridium difficile infection is a healthcare-associated infection that is increasingly affecting the healthcare institutions and people. It causes symptoms that can be mild or moderate, with fewer problems to the patient or a severe condition that causes complications and even death. To prevent this infection it is important to recognize the risk factors, its symptomatology, diagnostic tests, and the most appropriate treatment. The main objective of this study was to evaluate the Clostridium difficile infection’s costs in CHUA Medicine’s Department, in 2015. In this descriptive and case-control investigation, it was analyzed the existence of risk factors in people who developed the infection as well as the cost of medication and complementary diagnosis methods used by 45 individuals with infection (cases) and 45 patients without infection (controls). The results showed that patients with this infection had a hospital stay 4.1 higher than those without the disease, and the costs of medication, complementary diagnosis methods, and also the total costs were about 3 times higher in cases than in controls. The results highlight the awareness from health- professionals and managers in order to prevent the disease. Moreover, it is essential the existence of methods in the healthcare institutions able to individualize costs, by patient, and, consequently, to measure them.
