Abstract(s)
Benign prostatic hyperplasia (BPH) is one of the most common urological conditions in older men, characterized by the proliferation of stromal and epithelial cells in the transition zone of the prostate. This proliferation causes lower urinary tract symptoms, which negatively affects sexual function and quality of life. Although the etiology of BPH is not fully understood, factors such as increased dihydrotestosterone, inflammation, estrogens, genetic factor, insulin, and insulin.like growth factors may be implicated. The treatment of BPH aims to reduce symptoms, improve the man´s quality of life, and prevent future complications. Therapeutic options range from behavioral and lifestyle modifications to pharmacological treatments, including α1-adrenergic receptor antagonists, 5-alpha-reductase inhibitors, phosphodiesterase-5 inhibitors, muscarinic receptor antagonists, and beta-3 agonists. In more severe cases, surgical interventions may be necessary. However, these therapies are often associated with adverse effects related to sexual functions, especially in combinations therapies, which hampers adherence to the pharmacological treatment. Serenoa repens has emerged as an effective and safe therapeutic alternative for BPH. According to several studies, Serenoa repens extract has shown significant benefits in reducing urinary symptoms, thanks to its anti-androgenic, anti-inflammatory, anti-proliferative, and pro-apoptotic properties, which contribute to symptom relief and improved patient quality of life. Compared to conventional treatments, Serenoa repens has demonstrated similar clinical efficacy and is associated with a lower incidence of adverse effects, particularly regarding sexual function, which favors adherence to therapy. The most common side effects are gastrointestinal but are generally classified as mild, self-limiting and reversible. Thus, Serenoa repens appears as a promising alternative therapy in the treatment of BPH, particularly in situations where other therapeutic options are limited or present significant and uncomfortable adverse reactions for patients.
