- Défices cognitivos em pacientes com depressão unipolar: Influência das variáveis idade, género e história anterior de tentativas de suicídioPublication . Moniz, Emanuel Marco Fernandes; Jesus, SaulA depressão afeta mundialmente cerca de 350 milhões de pessoas, sendo considerada a principal causa de incapacidade a nível mundial, comprometendo seriamente a qualidade de vida, apresentado este estado afetivo um elevado risco de suicídio para estes doentes. No que se refere ao suicídio, o mesmo pode ser considerado um fenómeno global e um grave problema de saúde pública, responsável por mais de 800 mil mortes anualmente. Défices nas funções executivas, na memória e na velocidade motora são comuns na depressão e ainda mais exacerbados em indivíduos depressivos suicidas. Torna-se, por isso, essencial generalizar o recurso a meios complementares de diagnóstico, como os testes neuropsicológicos que medem a capacidade de decisão, de mudança de foco de atenção, de planeamento, de inibição de resposta e o controlo inibitório, entre outros, que, para além de fornecerem informações importantes para um correto diagnóstico e tratamento da depressão, propiciam estudar as dinâmicas subjacentes aos comportamentos suicidas, com vista a encontrar marcadores cognitivos que permitam identificar casos de maior risco visando o seu controlo e prevenção. Em Portugal, o recurso a medidas neuropsicológicas ainda não se encontra generalizado, por isso, um dos objetivos deste trabalho passou por validar um conjunto de testes computorizados de um software de livre acesso da Psychology Experiment Building Language (PEBL) junto da população portuguesa – a saber, Finger Tapping Task, Wisconsin Card Sorting Test, Torre de Londres, Iowa Gambling Task, Victoria Stroop Task e Go/No-go –, procurando simultaneamente estudar amostras de depressivos unipolares não-psicóticos. Outro objetivo consistiu em aplicar esses testes em pacientes suicidas com igual diagnóstico, procurando encontrar marcadores cognitivos que os diferenciassem de depressivos não suicidas, tendo-se corroborado a existência de défices nas amostras depressivas, com uma alteração significativa da inibição de resposta nos sujeitos suicidas face aos não suicidas e face aos controlo saudáveis.
- Molecular and functional analysis of DAND5 in human Congenital Heart Disease (CHD)Publication . Cristo, Fernando Jorge Pego; Belo, JoséThe majority of congenital heart disease (CHD) is sporadic, with a minority of cases associated with a known genetic abnormality. Combinations of geneticenvironmental factors are implicated in the etiology of the disease. Recently, several studies, using mostly animal models, unraveled that perturbations in the molecular processes that precede the beginning of heart development might also be at the origin of CHD. In fact, some of the most complex CHDs are found associated with laterality defects, a disorder resulting from abnormal Left-Right axis formation. In our laboratory, the identified mouse Cerberus-like2 (Cerl2 – human DAND5), a protein that inhibit Nodal signaling, prompt us to study cardiac and laterality diseases, since the generated Cerl2 KO mice display a wide range of laterality defects and CHD. Considering the high conservation of genetic pathways regulating cardiac development in mouse and human, the main objective of the present thesis was the study of human genes involved in the Nodal pathway, focusing mostly in DAND5, in a CHD and/or laterality defects patients cohort. The sequence analysis of DAND5 revealed two patients displaying the same p.R152H variant, resulting in a substantial decreased in the function of the protein. We propose that p.R152H acts as a risk allele for CHD and/or laterality defects. In addition, we found two alterations in NODAL, two alterations in PITX2C and one alteration in CFC1. We hypothesized that the NODAL p.H165R variant can act as a common modifier and the intronic variants in NODAL and PITX2C might cause alterations in the splicing pattern of the mRNA molecules. Moreover, we generated patient-specific iPSCs to understand the molecular mechanisms of disease behind the DAND5 nucleotide variant. Although we cannot make a clearly genotype-phenotype correlation, the variants here identified probably increase the disease susceptibility due to the resulting abnormal Nodal signaling. Because most of the patients presented more than one alteration, the cumulative effect of each variant within the pathway seems to enhance even more these risk. Therefore, the imbalance in dosage-sensitive Nodal signaling is a common denominator for laterality defects and associated CHDs.
- Rede nacional de cuidados continuados integrados do Algarve: perspetiva dos familiares, utentes e profissionais de saúdePublication . Duarte, António José Arsénio; Nunes, CristinaConhecer as perspetivas dos utentes, familiares e profissionais da Rede Nacional de Cuidados Continuados Integrados (RNCCI) é de grande importância para delinear estratégias de melhoria da prestação de cuidados à população com dependência. Neste trabalho foram realizados 6 estudos com o objetivo de aprofundar o conhecimento sobre a RNCCI do Algarve, tendo sido incluídos 152 utentes, 37 profissionais, 211 Cuidadores Informais (CI). De modo geral, conclui-se que: 1) Os utentes das Unidades de Média Duração consideram a falta de suporte familiar e a perda de autonomia as razões do internamento e realçam a necessidade de ajuda de terceiros no domicílio, de melhoria da qualidade dos serviços e de alterações no domicílio adequadas à sua capacidade funcional. As redes de suporte social dos utentes das unidades da RNCCI são compostas maioritariamente por familiares. Os utentes apresentaram auto-perceção e satisfação com a saúde razoáveis estando a necessidade de suporte social negativamente associada algumas dimensões da Qualidade de Vida (QV). Os utentes apoiados pelas Equipas de Cuidados Domiciliários (ECCI) percecionaram maior necessidade e satisfação no apoio emocional. A satisfação com o suporte emocional correlaciona-se com melhor QV. 2) Os CI de utentes das unidades de internamento da RNCCI reportaram níveis satisfatórios de QV apesar de apresentarem níveis elevados de sobrecarga. Idade, sexo feminino, baixo nível socioeconómico, viver sozinho, maior duração dos cuidados prestados e cuidar do conjugue, associam-se com maior sobrecarga e pior QV. Os CI de utentes apoiados pelas ECCI reportaram níveis elevados de sobrecarga e pior QV quando prestam cuidados mais horas diárias e realizam esta tarefa há mais anos. 3) Os profissionais das Unidades de Longa Duração identificaram défices de respostas sociais adequadas às necessidades dos utentes, de ofertas de trabalho e de preparação académica; destacaram o bom ambiente de trabalho e a autonomia profissional.
- Studies towards unveiling the association of Gla-rich protein with osteoarthritisPublication . Cavaco, Sofia Isabel Franco; Simes, Dina; Viegas, CarlaOsteoarthritis (OA) is a whole-joint disease believed to onset after articular cartilage damage and accompanied by tissue inflammation, abnormal bone formation and extracellular matrix (ECM) mineralization. Gla-rich protein (GRP), the latest discovered vitamin Kdependent protein (VKDP), was shown to accumulate in mouse and sturgeon cartilage, and sites of skin and vascular calcification in human. Therefore, we investigated the possible involvement of GRP with OA development. An osteoarthritic and control samples human biobank was collected and used for the comparative analysis of GRP patterning at transcriptional and translational levels. Two novel GRP alternative spliced transcripts were unveiled in human (GRP-F5 and F6), yet GRP-F1, corresponding to the full-length protein, was shown to be the predominant variant in articular tissues and upregulated in osteoarthritic cartilage. Undercarboxylated GRP was the prevalent protein form found associated with osteoarthritic cartilage and synovial membrane tissues, highly accumulated at sites of calcification, indicating that the impairment of VKDPs -carboxylation may be related with OA. Using a chondrocyte and synoviocyte cell system developed within this project, we further investigated the association of GRP with OA mineralization and inflammatory processes. Upregulation of GRP was found during induced mineralization and inflammation, and associated to cell differentiation towards ECM mineralization and inflammatory responses, in both cellular types. Moreover, the role of GRP was highlighted through functional assays, showing the inhibition of ECM mineralization and decreased inflammatory response following GRP supplementation. While -carboxylation was required for GRP antimineralization function, its anti-inflammatory effect was independent of protein - carboxylation status. Ultimately, using serum samples from our biobank and a comparative proteomic approach, candidate OA biomarkers were identified. Overall, our results demonstrated, for the first time, the involvement of GRP in two of the main pathological processes occurring in OA, contributing for new knowledge regarding disease progression.
