- O acesso aos serviços públicos de saúde no futuro: Elementos para a realização de uma antropologia antecipatóriaPublication . Pinto da Costa, José CarlosO documento discute como as transformações nos sistemas de saúde, como a medicina de precisão, saúde digital e cuidados baseados em valor, podem afetar a equidade no acesso à saúde no futuro. Apresenta as três discursividades e como contribuem para a eficiência, mas também podem aumentar os custos e dificultar o acesso. Conclui que é preciso antecipar os impactos destas transformações nas populações para promover a equidade no acesso aos cuidados.
- Anticholinergic burden in elderly people in nursing homes: cross-sectional assessment using ACB calculator and CRIDECO anticholinergic load scalePublication . Nascimento, Tânia; Matos, Maria Ana; Pinto, EzequielBackground/Objectives: Anticholinergic burden is an important risk marker in older adults, associated with cognitive decline, falls, and increased mortality. This study aimed to assess anticholinergic burden in institutionalized elderly individuals using two tools (ACB calculator and CALS—CRIDECO Anticholinergic Load Scale), as well as to analyze its relationship with pharmacotherapeutic variables like polypharmacy. Methods: A descriptive cross-sectional study was conducted by analyzing the pharmacotherapeutic profiles of institutionalized elderly individuals (≥65 years) utilizing individualized medication preparation services from a community pharmacy in Alentejo (Portugal). Participants agreed to the study and had complete, up-to-date pharmacotherapeutic profiles. Results: The pharmacotherapeutic profiles of 75 institutionalized elderly people were analyzed; the sample comprised mostly women (72%) who had experienced excessive polypharmacy (≥10 medications) (56%) and had an average age of 85.62 ± 7.62 years. It was found that 90.7% (ACB) and 89.3% (CALS—CRIDECO) of the elderly had anticholinergic burden, with mean values of 3.60 ± 2.84 and 3.33 ± 2.51, respectively. Women exhibited higher anticholinergic burden in unadjusted analyses (p < 0.05). The burden correlated moderately with the total number of medications (p < 0.05). Conclusions: The results show high exposure to anticholinergic medications in the institutionalized elderly population, reinforcing the rationale for systematic therapeutic reviews focused on the pharmacological safety of institutionalized older adults in community pharmacies.
- As Representações biomédicas da saúde e da doença tidas pelos enfermeiros no contexto multicultural da prestação de cuidados da UCCI da Santa Casa da Misericórdia de PortimãoPublication . ; Pinto da Costa, José CarlosEste trabalho resulta de uma investigação antropológica cuja finalidade consiste em encontrar a resposta para a pergunta: como é que os enfermeiros representam a saúde e a doença no contexto multicultural de prestação de cuidados da Unidade de Cuidados Continuados Integrados da Santa Casa da Misericórdia de Portimão? Para atingir este fim caracterizou-se o contexto multicultural da prestação de cuidados de enfermagem na Unidade e levantaram-se as representações que os enfermeiros possuem sobre a saúde e a doença, através de uma metodologia apoiada na observação participante e na realização de entrevistas semi-directivas aos enfermeiros. Observou-se que, apesar de acolher doentes estrangeiros, a Unidade não dispõe de qualquer forma de informação nos seus idiomas. As respostas dos enfermeiros às entrevistas mostraram que eles se apoiam na definição de saúde proposta em 1946 pela OMS para objectivarem a saúde e a doença, respectivamente, como bem-estar e como mal-estar. Desta definição, os enfermeiros reforçam a natureza orgânica da manifestação de doença, salientando a dimensão física do indivíduo em relação às psicológica e social. Este pendor traduz-se numa visão biomédica apoiada em constelações etiológico-terapêuticas que relevam a objectividade da doença e a centralidade do organismo como balizas para a interpretação da realidade do cuidado. Por força desta perspectiva, os enfermeiros têm tendência a olhar para o doente indistintamente, sem reconhecerem a diversidade psicológica e cultural das interpretações sobre a saúde e a doença. Daqui resulta que os enfermeiros não realizam um trabalho de adaptação do cuidado à nacionalidade dos doentes, impondo uma visão ética pretensamente universal às diferentes realidades émicas que encontram. Esta atitude traduz-se em desadequações ao nível do cuidado e da comunicação com os doentes, onde, a incompreensão da linguagem do doente estrangeiro e a presunção do consentimento emergem como principais problemas, que podem mesmo questionar a legitimidade do princípio de beneficência. Estes dados suportam o argumento de que as desadequações entre as representações da saúde e da doença tidas pelos enfermeiros e a multiculturalidade do contexto de prestação de cuidados da Unidade resultam da proximidade da enfermagem ao modelo biomédico, que interpreta a estrutura da realidade clínica como um cenário regulado pela objectividade e pelo reducionismo da vida à existência orgânica. Com efeito, os dados mostram que as representações da saúde e da doença tidas pelos enfermeiros são moldadas pelas instituições onde eles se fazem e se realizam e que são concebidas como lugares de produção da verdade absoluta, associando o saber à sua vontade de poder dentro de uma lógica de governalidade aparentemente exterior à biopolítica do Estado, transformando as suas políticas neoliberais multiculturalistas em práticas autoritárias assimilacionistas.
- Assessment of anatomical changes by pulmonary Angio-CT resulting from pulmonary thromboembolismPublication . B.PTE (Pulmonary Thromboembolism) is the third most common cause of cardiovascular death, with more than 600,000 cases per year in the US. This pathology has a wide spectrum of presentations and severity, with some patients experiencing shock, requiring urgent thrombolysis, while others can be safely treated on an outpatient basis with anticoagulation treatments alone (John, Marti Poletti, Perrier, 2014). The degree of pulmonary arterial obstruction and the previous cardiorespiratory status determine the hemodynamic consequences of PTE, the elevation of pulmonary pressure is insignificant until the obstruction of the pulmonary arterial bed is greater than 30–50%, whereby above this threshold it increases rapidly, causing an elevation of pulmonary vascular resistance and consequently significant pulmonary hypertension with enlargement of the proximal arterial vessels, resulting in an afterload on the right ventricle and therefore its dilation. (Bryce, Perez-Johnston, Bryce, Homayoon, & Santos -Martin, 2019; John, et al., 2014). The main objective is to assess the anatomical changes that may be present in patients diagnosed with PTE. In order to be able to reach the main objective of the work and answer the pre-defined research questions, there was a need to also define specific objectives, which consisted of analyzing which alterations were the most evidence of the presence of PTE; relate verifiable anatomical alterations through pulmonary Angio-CT with gender; relate verifiable anatomical alterations through pulmonary Angio-CT with age; to relate the curvature of the interventricular septum and the dilation of the right ventricle and also to relate the dilation of the pulmonary artery trunk and the dilation of the right ventricle.
- A autoetnografia como opção metodológica no estudo antropológico das situações de vulnerabilidade: exemplo de um caso de hipotiroidismoPublication . Pinto da Costa, José CarlosO antropólogo confronta-se, desde sempre, com a incapacidade de participar da condição das pessoas que vivem situações de vulnerabilidade, concretamente, devidas a problemas de saúde, ao sofrimento de situações de violência ou mesmo à experiência da clausura. Neste artigo argumenta-se que este ângulo morto da análise antropológica pode ser relativamente superado através de uma abordagem autoetnográfica. Descrevendo a sua experiência pessoal num caso de falência da função tiroideia, o autor fornece indicadores que poderão ajudar a analisar em profundidade as experiências das pessoas que vivem situações de vulnerabilidade.
- Bone mineral density and body composition among individuals who practice sports with mechanical impact and sedentary activitiesPublication . Pinheiro, João; Ribeiro, Luís Pedro; Coelho-e-Silva, ManuelThe purpose of this study was to evaluate indicators of bone health associated with sports practice in male adolescents using dual-energy X-ray absorptiometry (DXA) to measure bone mineral content (BMC), density (BMD), and body composition (BC) for the whole body (WB) and specific regions. This cross-sectional study involved 65 individuals (18–35 years). Athletes had higher BMD for WB (1.064 ± 0.121 g/m−2) than sedentary individuals (0.753 ± 0.129 g/cm−2). Differences were also observed within specific regions such as the trunk, upper and lower limbs. Individuals who practice regular sports present improved bone health indicators compared to their sedentary peers.

(2015) citing Bianchi ML. (2013) Cerebral Palsy (CP) is one of the most prevalent diseases within those that affect bone mass, and is often related to other pathologies such as osteoporosis. According to António, D., H., et al, (2020) citing NIH, (2002); WHO, (2003) osteoporosis is a pathology that involves others and is defined by the deterioration of bone tissue. In this way, with the alterations caused in the bone tissue and with the reduced BMD values, the bones lose resistance to traumas, causing their fragility and, consequently, a greater probability of the occurrence of fractures. In this way, BMD measurement will help in the diagnosis of osteopenia/osteoporosis and in the prevention of future bone fractures. In addition to bone densitometry, there are other methods, less invasive and less expensive, that effectively analyze BMD. CP is characterized as a permanent, though not invariable, disturbance of movement and posture due to a non-progressive defect or lesion of the brain early in life. (Rosa, A.C.A. & Matos, M.R. (2017)) Depending on the area of the nervous system affected, the patient with CP presents neuromuscular alterations. Each change will compromise differently and to different degrees, affecting the functional performance of these patients.
- Comparing bone density and body mass in male adolescents: A contrast between soccer players and swimmersPublication . Pinheiro, João; Ribeiro, Luís Pedro; Silva, M.Physical activity seems to promote and benefit bone mass gains during growth for specific regions of the skeleton. Soccer has greater impact on specific regions of the pelvis and swimming on the upper limbs. Efforts should be made to encourage modification of all risk factors for osteoporosis early on life such as government policies to promote sports practice.
- Early identification of bladder diverticula in pre-hospital settings using screening sonography: a case reportPublication . Miravent, Sérgio; Gomes, Carla; Basescu, Andrea; Martins, Ana; Lobo, Manuel Duarte; Almeida, RuiObjective: Bladder diverticula, while not exceptionally rare in clinical practice, presents a significant diagnostic challenge, particularly in remote pre-hospital emergencies where technological limitations and the absence of specialized medical expertise prevail. This report aims to emphasize the role of screening ultrasound in the pre-hospital setting, Case Presentation: A patient experiencing severe lower abdominal pain presented at a basic emergency service (BES) for the third time presenting the same symptoms. Renal and pelvic ultrasound screening was performed, revealing suspected bladder distention and adjacent fluidfilled cavities, consistent with multiple bladder diverticula. Subsequently, the patient was referred to a referral central hospital for management by medical experts. Comprehensive evaluation confirmed the findings of the BES. Conclusion: The results of the screening ultrasound enabled the BES team to initiate clinical interventions to mitigate the patient’s discomfort. Screening ultrasound was crucial in identifying imaging markers that provided the physician with clinical data for correct and prompt patient orientation.
- Editorial: Innovative teaching and learning in health education and promotionPublication . da Costa, Emilia; Ruiz Fernández, María Dolores; Fernández Medina, Isabel María; Jimenez Lasserrotte, Maria del Mar; Ventura-Miranda, Maria IsabelHealth education and health promotion are undergoing profound transformation. Demographic transitions, aging populations, increasing multimorbidity, persistent inequities, and rapid technological change are reshaping how learners understand and navigate health. In this evolving context, traditional knowledge-transmission models are no longer sufficient to prepare future professionals for complex, multicultural and digitally mediated environments. Innovation in teaching and learning has therefore become essential, not only to improve learning outcomes, but to strengthen ethical reasoning, equity, and learner autonomy. This global shift echoes recent OECD (1) analyses highlighting how digitalisation, demographic aging and widening social disparities are redefining the competencies required of tomorrow’s health workforce and calling for educational approaches attuned to complexity and uncertainty. It is also consistent with the World Health Organization’s call for transformative health workforce education, which stresses that conventional training models can no longer meet the demands posed by demographic change, chronic disease burdens, technological acceleration and growing inequities.
